Location: Teesside / Redcar and Cleveland
Responsible to: Operations Manager
Hours: 36 hours – Fixed term till 31st March 2020 with possible
extension of contract until 2021 and beyond.
Salary: £16,848 per annum

Job Purpose

To assist in the delivery of Age UK Teesside’s integrated services. This is a split post working 21 hours assisting with the Befriending Service and 15 hours running the Welcome Home Project. Both posts will be based within Redcar and Cleveland.

Job Description – Befriending Service

To work alongside the Befriending Services Co-ordinator and assist in the assessment of new and existing client’s requirements around befriending and help facilitate their engagement within new networks to reduce the effects of loneliness and social isolation. They will also be responsible for developing and promoting the Befriending Service’s Extended Service by working in close partnership with other local services.

To develop a strong knowledge and working relationship with external providers and services to ensure clients have access to choice.

Job Description – Welcome Home Project

To deliver a safe and reliable ‘Welcome Home’ project for vulnerable older people, who may live alone and who are returning home following a stay at the Recovery and Re-ablement unit who are from the Redcar and Cleveland area

Overall Aims: To reduce re-admission rates of older lonely people following a hospital stay; to increase individual’s well-being and independence; to reduce levels of loneliness and social isolation

To attend and participate in internal/external meetings, as appropriate

Under the Health & Safety at Work Act 1974 and associated guidance, it is the duty of all staff while at work to take adequate care for the health and safety of themselves and of other persons who may be affected by their acts or omissions.

To work with other staff to ensure appropriate customer care is provided at all times.

To develop a personal knowledge base on Age UK Teesside to underpin effective working.

All staff are required to work within Age UK Teesside’s policies, ensuring these are carried out in relation to the job e.g. confidentiality, equal opportunities etc.

The post may involve occasional evening and weekend work for which time in lieu can be claimed.

A full driving license and access to a vehicle will be essential to carrying out the duties of the role.
