Dr Vaishali Nanda, local GP and Middlesbrough Locality Lead for NHS South Tees CCG announced the fund on 26th September 2013 at the Middlesbrough Voluntary Development Agency Health and Social Care Group and said, “It is hugely important for the CCG to work with our local groups and organisations. We must embrace innovation in order to meet the expectations of patients during these challenging financial times and we know that there is a wealth of untapped ideas, knowledge and experience within our community. This fund is a key milestone for the CCG for both its partnership approach and its commitment to innovate for the benefit of local people”.
The fund has been established to enable local groups/organisations to promote health and make a difference to the people of South Tees lives and has a total value of £200k. The scheme is aimed at any community organisation, patient groups, and charities etc. who wish to apply for a proportion of the fund to benefit the South Tees area. Bids can be placed for up to a maximum of £40K.
Funding is available for organisations to develop new and innovative areas of work that have a positive impact on health and wellbeing and support one or more of the CCGs priority areas:
- Health and wellbeing e.g. to support carers; improve uptake and screening or self-management of Long Term Conditions (LTC).
- Urgent Care e.g. preventing hospital admissions and getting people back home swiftly
- Care Closer to Home e.g. enabling people to receive care and remain close to home
- Planned Care e.g. improving pathways and patient journeys, making best use of resources
- Mental Health and Learning Disability e.g. improving uptake of health action plans and health checks, suicide prevention and supporting people with dementia
- Medicines Optimisation e.g. reducing waste and helping people better manage their own care
- Quality improvement in primary care e.g. improving the quality of care provided in general practice

Applications can be submitted from 26th September 2013.
The closing date is 12 noon on 25th October 2013.
The funding will be released by the CCG no later than the end of this financial year.
